An illustrious group of reputable Indian plastic surgeons and medical experts gathered on April 7th at A1-10 / Prashant Vihar, New Delhi. The reason for that occasion was the inauguration of the new hospital complex of OLMEC Healthcare, Dr. Sunanda Kaushik and Dr. Narendra Kaushik‘s youngest “child” which officially opened its doors to the public that day.

Dr. Narendra Kaushik and Dr. Sunanda Kaushik were filled with pride and felt blessed that all of Dr. Kaushik‘s former teachers from primary school, high school, and medical
school, as well as his attending medical professionals, took their time to come to Delhi to celebrate with their former student and intern who nowadays is one of the most well known and highly rated plastic surgeons in India.

Dr. Kaushik‘s heartfelt gratitude goes out first and foremost to the most important teacher of all, his father Shri Jagdish Chander Shastri who instilled the values of discipline,
honesty, hard work, and the quest to constantly improve in his son.

Among the many guests, Dr. Kaushik was very thankful and pleased to have Dr. Roshni E. Rana, retired head of the plastic surgery department of the prestigious K.E.M. Hospital Mumbai as the main guest who was mentor and guide to Dr. Kaushik for his MCH years.

Dr. Kaushik also thankfully welcomed his further MCH mentors Dr. Vinita Puri, current head of the plastic surgery department of K.E.M. Hospital Mumbai, and Dr. Amresh Baliarsing, current president of the Association of Plastic Surgeons of India. Present from PGIMS, Rohtak was Dr. Sham Singla, his guide and mentor during his MS years at Rohtak. Also present were Dr. Kaushik‘s high-school physics teacher Ramesh Aggarwal and his primary school teacher Satyanarayan Yadav. The event was further graced by the presence of Dr. Kaushik‘s father-in-law, retired director of the Haryana Government Health Services, and senior gynecologist Dr. D. K. Bhardwaj who imparted his extensive urogenital knowledge to Dr. Kaushik.

Psychiatrist evaluations for Gender Dysphoria:

Before starting the transition process it is mandatory to get the “Gender Dysphoria” Diagnosis from a psychiatrist. Usually, an experienced psychiatrist will diagnose “Gender Dysphoria” in a single sitting and will clear you for the further treatment, but if required, there can be multiple sessions for the same. Single Session with a Good Psychiatrist will cost around 2000 – 3000 INR. Cost for Psychiatrist evaluation is different in different cities, the price mentioned above is for metro cities.

After the psychiatrist evaluation now it is time for Hormonal Therapy. Hormonal Therapy is again the important part of the transition process. Hormonal therapy is as essential as psychiatrist evaluation. After 6 months of HRT, you again have to visit the psychiatrist and one other mental health therapist for the clearance of the SRS Surgery, this two visits to the mental health therapist will cost you around 5000 INR.

Hormonal Therapy: Endocrinologist evaluation

Hormonal therapy in male to female and female to male transition necessary, before any procedure it is compulsory to have hormonal therapy prescription. Moreover, it is necessary that you take hormones for rapid transition. Before starting HRT you have to consult Endocrinologist, Cost visiting endocrinologist is around 1000 INR, Which is again different in different cities. You have to purchase the medicinal hormonal that are prescribed by an endocrinologist. The cost of purchasing hormones in Male to Female is around 2000 INR per month and for Female to Male, it is around 600 INR. In MtF and FtM transition, the minimum duration of hormonal therapy is 6 months. Once the Hormonal therapy is completed, you have to again visit the mental health therapist for the clearance of the surgery.

Does OLMEC also perform penile inversion GCS/SRS?

The answer to that is no.
The simple reason for this is that our own SIGMA-LEAD GCS/SRS technique uses a rectosigmoid colon graft to create the neovagina. No penile or scrotal skin is used for that purpose since they don‘t meet the demands of a vagina and are way too hard to maintain post-op.
However, some of the steps we use throughout our 2-stage one-time surgery are similar than with penile inversion approaches.

The vaginal cavity is created by carefully dissecting the area between rectum and bladder. Dissection is continued all the way to the peritoneum. In the penile inversion technique, the peritoneum is left intact.

In a regular penile inversion approach, the penile skin gets pulled down as much as possible to line the vaginal cavity with the largest available amount of penile tissue possible. It‘s understood that this tension is counterproductive in so many ways, as well as using valued penile skin for the vaginal lining.

This is where Dr. Kaushik‘s approach goes different ways. In a refined procedure, he uses pedicled tissue (with nerve and blood supply) to create the clitoral hood and the labia minora as well as the inner side of the labia majora.

He also performs a well-refined clitoroplasty that changes the glans into a clitoris of natural size and shape. He then positions the urethra at the proper location, just like in a biological female. Remaining penile skin will be attached absolutely tension free at the vaginal entrance. This is what Dr. Kaushik calls stage 1 of the SIGMA-LEAD GCS/SRS technique.

The patient has the choice of leaving it as it this being cosmetic GCS/SRS or stage 2 of the SIGMA-LEAD GCS/SRS will follow immediately, or at any later date, should the patient choose to do so. Yes, you can opt for the second stage also at a later time.

As you can see, this procedure has only very few elements of a penile inversion surgery in common. That is also the reason why it has a different name.
The steps involved in the OLMEC cosmetic GCS/SRS are far too advanced and different from regular penile inversion approaches.
